1.  I don't know.  I was three or four when I caught my first fish. 

2.  Zebco pistol grip fiberglass rod and Zebco 606 spin caster.

3.  Daiwa blue fiberglas 7'6" ML spinning rod I bought at a grocery/department store in Ohio.  Might have been Heilig Meyer.  Paired it originally with an Olympia, but the perfect reel for it turned out to be a Daiwa 250RL I bought at a going out of business or closeout sale at a Yellow Front store in Prescott Arizona.  I caught everything from tiny pan fish to stocker trout to 15 pound stripers with that rod and reel.
